Enrollment/Withdrawal Process

Welcome to Pembroke Middle School: "Home of the Warriors" where our students excel with pride in academics and athletics. Your child is important to us and we are committed in providing a quality educational experience which includes high expectations for all students. Please review the following guidelines which are necessary to enroll or withdraw your child. This procedure will allow a smooth transition from school to school.

Enrollment guidelines:

  1. Enrollment process must be attended by the adult who has legal custody of the student.

  2. Student must be officially withdrawn from the previous school.

  3. Documents needed:

    1. Birth certificate

    2. SS card

    3. Picture Id (of the adult who is enrolling the child)

    4. Immunization record

    5. Withdrawal records from previous school

    6. Proof of residence

    7. Custodial paperwork

Withdrawal guidelines:

  1. Withdrawal process must be attended by the adult who has legal custody of the student.

  2. Student must return classroom books, library books, and calculator.

  3. You will receive the necessary documentation to enroll in the transferring school.

  4. Students should be enrolled in the transferring school as soon as possible (same day if possible).
